He Avoided Surgery and Got Back on the Bike. Here's How the Right Referral Made It Possible.

Juan Carlos came to FISIOPR on his own — he filled out a form, described what he was going through, and our team took it from there. What he was experiencing pointed to something that needed a closer look before we could responsibly build a program around it. FISIOPR referred him to a specialist. Dr. Ricardo J. Fernández-de Thomas, MD (a neurosurgeon) evaluated him, confirmed the diagnosis, and referred him back. That loop, from patient to specialist to program, is exactly how it's supposed to work.

The diagnosis involved lumbar pathology with sciatic-like symptoms. Walking had become painful. Cycling (the one thing he loved) was off the table. Surgery was on the table, but Juan Carlos wasn't ready to go that route. Neither was his physician at that stage. The decision was made to pursue a structured conservative approach first.

At FISIOPR, his program began with a lower back reconditioning protocol — individualized, progressive, and built around where he was, not where a generic protocol assumed he should be. The early focus was symptom management and movement restoration. Week by week, the program advanced as his body responded really well.

Eight weeks in, the results were clear. The sciatic-like presentation had reduced significantly. His confidence in his body came back. And cycling (the goal from day on) was now back within reach.

Juan Carlos did not need surgery. He completed the program, recovered the capacity to manage his own training going forward, and left with a body that worked better than when he came in.

What made this case work was the system around it. FISIOPR did not try to skip steps. When the clinical picture required a specialist, we made that referral. When the specialist cleared him for reconditioning, we had a program ready. Juan Carlos was never left in a gap between providers wondering what to do next. He had a path, and people who knew how to walk it with him.

FISIOPR programs are designed for people who want to get better and have moved past the acute stage. People who need structure, follow-through, and someone who knows what to do day after day. They adapt to each person's context.

This is one of the many cases we want to serve. FISIOPR wants to impact 100,000 lives. Help us keep making a difference in our community.
